Scheduling Your Appointment

Call 770-6776 or toll free 800-681-6776.

To schedule an appointment, please call the office during business hours (9:00 am to 5:00 pm). Our patient coordinator will send you information on the procedure(s) that you are interested in. You'll also receive some forms that you should fill out prior to your visit.

Patients are seen by appointment only.

Of course, emergencies will always be given priority and are worked into our daily schedule. If you are unable to keep your appointment, please notify the office at least 24 hours in advance so other patients can be scheduled. We value your time and will make every effort to see you at your scheduled time. However, complex cases or emergencies may occasionally cause unavoidable delays. On these occasions, the receptionist will alert you of any significant wait.

Your Initial Consultation

Learning about Cosmetic Surgery.

When considering cosmetic surgery, the initial consultation is an important part of learning about the procedure you are considering.

At The Center for Cosmetic & Plastic Surgery, we require a consultation so that together we can identify your goals and expectations regarding the outcome of cosmetic plastic surgery. We will answer your questions, discuss your fears and concerns and make recommendations about the appropriate procedure for you. Our patient coordinator will discuss fees with you, including fees for your particular procedure and cost estimates for the surgical facility and anesthesia services. You'll also receive copies of the surgical consent forms, as well as preoperative and postoperative care instructions.

Fees and Insurance

Payments made easy.

Since we too, are concerned about the cost of quality medical care, we will do everything possible to keep our fees reasonable. Our billing and payment policies are summarized below:

Cosmetic Surgery

Cosmetic surgery is rarely covered by insurance. Some procedures, however, may be tax-deductible medical expenses. Payment is required two weeks prior to surgery. This reduces administrative expenses and helps to contain medical costs. For your convenience, we also accept personal checks, cash, MasterCard and VISA.

Non-cosmetic Surgery

Whenever possible, surgical procedures will be billed to your insurance company. It is your responsibility to pay any deductible, copayment or any balance not paid by your insurance company.

Travel and Accommodations

We'll make all the arrangements.

The Center for Cosmetic & Plastic Surgery is conveniently located just minutes from the Medford International Airport. Dr. Jensen's patients receive special discounts at a number of fine, local hotels/motels. Please call our patient coordinator for more details.
